The Official State Gazette (BOE) has published the new aid system approved by the Generalitat of Catalonia (GenCat) that will allow people with extreme dependency to collect up to 14,788.50 euros per month to finance home care and personal assistance. This measure is obtained in Decree-Law 1/2026 and creates a reinforced level of protection that is mainly intended for patients with advanced ALS and other highly complex irreversible diseases.
The new regulation has developed the so-called Grade III+ of extreme dependency, a category recently incorporated into state legislation and which expands public coverage for those who need continuous and permanent care 24 hours a day.

The Catalan Government has justified this measure as a way to offer a “rapid, flexible and adequate” response to people with rapidly evolving diseases, whose situation can worsen in weeks.
What the new aid covers
The new aid is designed to cover two types of support, such as the home help service and the hiring of specialized personal assistants. The decree establishes that this aid would become the main benefit for people with degree III+ of extreme dependency.
Catalonia expands the state maximum approved in 2025 by adding a regional supplement of up to 4,929.50 euros per month. The total amount will depend on the number of hours of care that are recognized in the Individual Care Program (PIA) that evaluates the level of support necessary in each case.
Requirements to request the provision of dependency aid
The aid that the BOE has already published is aimed at people who have recognized degree III of dependency, suffer from ALS in an advanced stage with total dependence for basic activities or suffer from other diseases or highly complex irreversible processes that are included in state regulations.
The decree mentions situations that require respiratory assistance, continued support, intensive personal care or permanent care due to dysphagia or loss of autonomy.
Criteria are also set according to the economic capacity of the beneficiary.
- People with income up to 5.5 times the IRSC may receive 100% of the benefit.
- Those who exceed that threshold will receive 95% of the recognized aid.
The Generalitat clarifies that the amounts must always be adjusted to the individualized evaluation of needs.
How much money can be charged
The maximum amounts are set based on the number of daily hours of recognized support.
- 8 hours of daily support: up to 4,930 euros per month
- 12 hours of daily support: up to 7,394.25 euros per month
- 16 hours of daily support: up to 9,859 euros per month
- 20 hours of daily support: up to 12,323.75 euros per month
- 24 hours of daily support: up to 14,788.50 euros per month
The maximum figure of 14,788.50 euros per month will only apply to cases with a proven need for permanent care throughout the day.
Compatibility with other aids
One of the most relevant changes is that people with grade III+ will be able to make all the benefits of the Catalan dependency system compatible.
Until now there were limitations between different aids and services, but the new decree eliminates these incompatibilities for this group, always within the maximum established economic limits.
This will make personal assistance compatible with day centers, occupational services, home help and other social supports, eliminating the restrictions that existed until now for people with extreme dependency.
